In principle, you do not need to submit a declaration to receive the benefit. The IT system of the e-Health Center is a system from which the Social Insurance Institution obtains information about the imposed quarantine or isolation and places it on the Electronic Services Platform (PUE ZUS) . It is seen both by the insured and his employer. In many cases, this information is enough for an isolated sick person or staying in quarantine to receive the remuneration he is entitled to for the time of illness or sickness benefit. However, if there is no information on the PUE ZUS platform, an employee should submit a short statement to his/her employer within 3 days.

If you are in quarantine or are ill in isolation, first of all, inform your employer as soon as possible. You can do it, for example, by phone or e-mail. You will not need a sick leave. You also do not have to deliver the paper sanitary inspection decision to the employer or to the Social Insurance Institution.

The tab is visible only when quarantined or in isolation. Thanks to this, the employer has knowledge of about the duration of the isolation / quarantine of their employees, sees the start and end dates, also information, e.g. about extending the quarantine or isolation. Data in PUE ZUS may also be verified by the insured himself. In many cases, this information is enough for an isolated sick person or staying in quarantine to receive the remuneration he is entitled to for the time of illness or sickness benefit. However, in a few situations it will be necessary to submit an appropriate declaration.
When is a declaration needed?
In the following cases:
- when there is no information on the quarantine / isolation of the employee on the PUE ZUS platform,
- in the case of an employee caring for a sick child,
- when an employee is in quarantine due to the isolation (illness) of a household member.
If there is no information in the PUE ZUS system that you were in quarantine or isolation, you should submit a short statement to your employer within 3 days from the end of compulsory quarantine or isolation at home. In the statement, enter:
- first name and last name,
- PESEL number of the insured person (if you have one),
- series and number of the passport, if it was presented to a Border Guard officer as part of an inspection,
- the date on which compulsory quarantine or isolation at home begins and the date it ends,
- signature of a person who has undergone compulsory quarantine or isolation at home.
The employer or the Social Insurance Institution may apply to the body of the State Sanitary Inspection for confirmation of the information contained in the declaration.
If your child is sick with COVID-19 and is in home isolation, you are entitled to a carer's allowance for the care you take while you are sick. You are entitled to unemployment benefit if you are caring for a child under the age of 14. If your child has a certificate of significant disability or a certificate of disability, including the following indications: the need for permanent or long-term care or assistance of another person due to the significantly limited possibility of independent existence and the need for constant daily participation of the child's guardian in the process of its treatment, age limit is 18 years old. You can apply for the allowance also when you are caring for a sick or other family member.
In order to receive the benefit, you should submit a declaration which will include:
- first name and last name,
- PESEL number, if you have one,
- series and number of the passport, if it was presented to a Border Guard officer as part of an inspection,
- the date on which compulsory quarantine or isolation at home begins and the date it ends,
- signature of a person who has undergone compulsory quarantine or isolation at home.
You are entitled to sickness benefit also if you were quarantined due to the illness of your household member . This is another case where you should make a statement in order to be paid out. Remember that in this situation, the statement should contain:
- name and surname of the insured person,
- PESEL number (if you have one),
- information about the date of commencement of compulsory quarantine and the date of its completion,
- data on a person living together or running a shared household with SARS-CoV-2 infection, i.e. name and surname, PESEL number, if any, date of commencement of compulsory quarantine or isolation at home and the date of its completion,
- the insured's signature.
In the case of a quarantine imposed due to an illness of a household member or caring for a sick child, there is no specific time limit for submitting the declaration. Remember that sickness benefits are only available to people who are covered by sickness insurance (e.g. in connection with employment under an employment contract, contract of mandate or running a business).
